Candidates who have been qualified for admission to the first year of study are required to submit a set of required documents to the Recruitment Section by September 30, 2026:

•    Personal questionnaire – printed from the candidate’s online account.
•    Certified copy of identity document, verified as true to the original.
•    Original or notarized copy of secondary school diploma and transcript, with legalization/Apostille, accompanied by a translation into Polish or English. 
•    A statement from the Director of NAWA confirming recognition in accordance with current regulations – if applicable.
•    Proof of English language proficiency at B2 level, in accordance with the Regulation of the Minister of Science and Higher Education of 2025.
•    Health certificate – template available for download in the candidate’s online account.
•    Health insurance policy.
•    Certificate of vaccination against hepatitis B.
•    Certificate of current MCAT score – required for U.S. citizens or U.S. permanent residents.

Failure to submit documents by the deadline will result 
in the removal of the candidate from the list of initially qualified candidates
